- This output was based on a workshop hosted by the attributed individual and co-editor at Liverpool University in 2012. It was jointly conceived and carried through by both of them. They were responsible for the selection of speakers at the colloquium, the identification of those contributions to be developed for the published volume, and for all stages of editing. All chapters received extensive and close comments from both editors, on style, content and details of argumentation. The introduction (pp. 1-19) was jointly planned, initially drafted by the attributed individual, with additional contributions and refinements from the co-editor. The attributed individual also was the sole author of chapter 10 (pp. 244-73). The attributed individual took overall responsibility for correction of proofs and the co-editor for the production of the indexes.
