- This work was co-edited by the attributed individual and one other. It has its origins in an international conference in Odense in 2009, co-organised by the editors. In subsequent years, the co-editors met three further times (once in Odense and twice in St Andrews) to work on the editing process. An initial stage in this was their selection of 11 submissions for elaboration for consideration in the volume. They provided recommendations for journal publication to the authors of work that could not be included.
All draft chapters were twice returned with recommendations to authors; two chapters were returned three times. The co-editors co-wrote the introduction (15pp) at 50% each, and had joint responsibility for compiling the bibliography and index. The attributed individual is the sole author of chapter 4. The attributed individual also provided dedicated English language proofreading for chapters 1, 9 and 10.
